Checkbox Selection on a form

Hi All,

i have a requirement on AX form , quite clueless on how to achieve it.please help me on this

from the above screen shot professional Service(column) is a checkbox on the form  , user should be able to select only one checkbox ( i.e column) 

currently i'm able to select all the checkboxes , but at any given time only checkbox should be selected
